Sarah is a 41-year-old single woman who lives alone and is employed as a nurse at a general hospital. She is currently on sick leave related to her struggles with depression. She has never been married and does not have children. At the first appointment she stated,“I spend a lot of time alone.” Her main concerns are described as overeating and difficulty controlling her weight. Her body mass index (BMI) is 43 kg/m2. On a typical day, her eating is quite controlled while at work, but after work she experiences intense urges to overeat and usually stops for large quantities of takeout food on the way home. She states,“I feel like sometimes I have no control over my eating or my weight, and this makes me feel horrible about myself.” Sarah also speaks of difficulties with assertiveness, particularly at work, and with social anxiety.
